Queen City Elder Care was represented by Co-owner, Mark Vollmer and Nationally Certified Activity Director, Kim Weber, ADC at the annual TriHealth Primary Care Physician and Care Coordinator Vendor Fair held at Bethesda North on April 4th, 2017.

TriHealth was selected as a test site for a demonstration project funded by The Centers for Medicare and Medicaid to evaluate the impact on costs and quality of care through enhanced communication among care professionals, institutions as well as patients and their families, while planning for and delivering care.  The initiative was launched in 2012 that designated participating primary care physician practices within the TriHealth system, including physician groups, hospitals and clinics throughout the Cincinnati metropolitan area to participate in the study.  The desired outcomes were to improve care provided to the patient by eliminating unnecessary or duplicate tests or procedures, through enhanced distribution of the patients’ medical history and profile so that ultimately the timeliness of appropriate care is enhanced along with a reduction in overall costs.  The model is dependent on many players coordinating information and resources, including insurance companies and government payors, alongside the medical delivery institutions and professionals.  A key component to the success of the model is the Care Coordinator position, which is staffed by nurses who follow patients throughout the arc of services from the primary care physician practices to the medical specialists and institutions including hospitals, clinics, skilled nursing facilities, assisted and independent living campuses and in-home care providers.  The role of the care coordinator is crucial to the success of keeping each patient on their care plan course, including proactive follow up visits, rehabilitative therapies, medication schedules and ongoing support from community resources and services, including non-medical home care.
